| To Register for a Goal-Line Soccer Clinic in your area, ....... | ||

you already know the Name and Number of the camp you wish to register for
please go to Step 2 below. Otherwise check out the Camp Schedule in this web site for the camp number and name of the camp
in your area.
For most camps you may deduct $10.00 per person from the listed tuition fee if you register on or before May 15th You may deduct an additional $ 5.00 for each immediate member of the family who you are registering (up to 3 family members) or an additional $ 5.00 if your child is registering for more than one Goal-line Camp. For example if the tuition fee is $130 and you are registering two family members on or before May 15th the tuition fee per family member is: $ 130 minus $10.00 minus $ 5.00 = $ 115.00 If you are registering two family members after May 15th the tuition fee per family member is: $ 130 minus $ 5.00 = $ 125.00

you wish to pay by credit card, go to Step 3. To pay by personal check please complete and print out the the Registration Form. Be sure to enter the camp number and name and sign the Medical Consent and the Liability Release Form. Mail your Registration Form and Check to: Goal-Line
